What it is: It's a meeting held at the end of each sprint, where the Scrum Team reflects on the past sprint, discusses what went well, what went wrong and what can be improved.

How it works: Usually it's facilitated by the Scrum Master. It starts with a positive round, discussing what went well. Then, the team discusses what went wrong, without blame. Lastly, actions for improvements are agreed and owned by team members.
