The best event to discuss technical debt that has accumulated and is affecting the product's performance is the 'Sprint Retrospective'. This is because the Sprint Retrospective is designed to enable the Scrum Team to inspect itself and create a plan for improvements to be enacted during the next Sprint. The purpose of the Sprint Retrospective is to make visible the adaptation of the done work for the upcoming period. So, it is a perfect occasion to discuss technical debt and make a plan to deal with it. It's less fitting to address this issue in the 'Sprint Review' as its primary focus is on the product itself and the feedback from the stakeholders. The 'Sprint Planning' is not an appropriate time either because here the focus is primarily on planning the work for the upcoming sprint based on the Product Backlog items and it doesn't address the problems encountered in the past sprint. Lastly, discussing this problem 'During the current Sprint' isn't the best approach since it could distract the team's focus from meeting their Sprint Goal.

The best action when a team consistently fails to meet their committed sprint goals during sprint planning would be to 'Re-evaluate the capacity of the team and adjust the commitment accordingly'. In Agile, it is essential to understand the team's true velocity and capacity, and plan the commitment for each sprint accordingly. This ensures the work is reasonable and realistically achievable, without overburdening the team. Adding more team members to increase productivity may cause additional delays due to the time it takes for the new team members to integrate and catch up, which is known as Brooks' Law. Extending the sprint length is not an ideal solution, as it may lead to decreased productivity, burnout, and the risk of unexpected changes or issues arising during the extended time. Reducing the quality standards to deliver on time is also not advisable, as it undermines the value of the work done and has the potential to lead to further issues down the line.

In the context of Scrum, the Sprint Retrospective is the appropriate event to review and analyze a situation where the Development Team was unable to complete several items in the Sprint Backlog by the end of the Sprint. The purpose of this event is for the Scrum Team to reflect on the past Sprint, inspect how it went, and identify actions for improvement moving forward.

On the other hand, Sprint Planning is focused on establishing the plan and goals for the upcoming Sprint, Sprint Review is held to inspect the increment and adapt the Product Backlog if necessary, and Product Backlog Refinement is an ongoing process of adding details, estimates, and prioritization of items in the product backlog. These events are not focused on analyzing past challenges or failures.
